We are seeking a Connecticut-licensed Nurse Practitioner for a virtual role. In this position, you will conduct telehealth visits, provide proactive healthcare guidance, and collaborate with our interdisciplinary team to ensure excellent patient care. This role offers flexibility in scheduling and the opportunity to work remotely on an hourly basis, with the possibility to transition from part-time to full-time based on availability and organizational needs.
As part of the role, you will receive training in obesity care management from one of the leaders in obesity care, Jamie Kane, MD. Jamie is one of the co-founders and Chief Medical Officer of Luro Health.
Key Responsibilities:
In this role, you will:
Telehealth Visits: Conduct real-time video visits and follow-up consultations for a variety of healthcare services, including chronic care management, remote patient monitoring, and urgent care needs.
Care Coordination: Collaborate with our care team and our partner’s care teams (physicians, nurses, dietitians, behavioral health specialists, and health coaches) to develop comprehensive and individualized care plans.
Patient Education & Support: Provide health education and counseling to patients and caregivers, helping them navigate treatment options and manage their conditions effectively.
Clinical Documentation: Maintain accurate, up-to-date electronic health records (EHR), including patient histories, treatment plans, and follow-up recommendations.
Prescription Management: Evaluate patient needs and prescribe appropriate medications as needed, adhering to state regulations and clinical guidelines.
Quality & Compliance: Follow clinical protocols, best practices, and relevant regulatory standards to ensure the highest quality of care.
Technology Utilization: Leverage telehealth platforms, communication tools, AI based platforms, and EHR systems to deliver efficient and effective virtual care.
Requirements:
Licensure: Active and unrestricted Nurse Practitioner license in the State of Connecticut (required).
Education: Master’s or Doctoral degree from an accredited Nurse Practitioner program (required).
Certification: Board-certified as a Nurse Practitioner (e.g., ANCC or AANP) in a relevant specialty (Family, Adult, Geriatric, etc.) (strongly preferred).
Experience:
Minimum of 1 year of clinical experience practicing independently as a Nurse Practitioner (required).
Telehealth or virtual care experience (preferred).
Experience in obesity medicine, nutrition and lifestyle modification (preferred)
Medicare and Medicaid care delivery experience (preferred)
Technical Skills: Comfortable using telehealth platforms, EHRs, Google Workspace (e.g., Gmail, Docs, Sheets), or similar software tools.
Other Requirements:
Ability to manage patient interactions effectively via video and phone
Excellent communication, problem-solving, and organizational skills
A reliable internet connection and private, HIPAA-compliant workspace at home
Schedule & Compensation:
Hourly Rate: Competitive and commensurate with experience.
Hours: Flexible, with the opportunity to transition from part-time to full-time based on patient volume and your own availability.
